HSG Credentialing

Per the HSG Charter, the HSG is an independent, non-profit organization comprised of academic and research clinical investigators organized for the principal purpose of conducting controlled clinical trials in Huntington disease (HD). The HSG aims to advance knowledge about the cause(s), pathogenesis, and clinical impact of HD and related disorders, and to develop and implement scientific strategies to examine promising therapeutic interventions. To be eligible for consideration for a clinical trial, the investigator, coordinator, and site must be credentialed. The HSG Credentials Committee has developed credentialing policies and application processes to ensure all HSG sites comply with HSG standards of inclusion.

Before filling out an application, please read through it and make sure you have all required elements ready to submit at one time. This application is not a “save as you go” process, so please be sure you have your necessary documents ready to upload when you complete your application.

Credentialing Requirements: INVESTIGATOR – Apply Now!

  • Degree – MD, PhD, NP, DO, PA (see below for non-physician policy and application)
  • Provide General Information, Site Information, Experience and Training on the application form.
  • Must have experience as an Investigator or Sub-Investigator in a clinical trial.
    • If the Credentials Committee finds that the applicant does not have sufficient clinical trial or HD clinical care experience, additional training suggestions will be recommended to the applicant.
  • Must have good standing tenure at an established site with the infrastructure necessary to run an HSG trial.
  • Must have completed the CME4HD lecture series training (in-person or online)
  • The following materials must be uploaded with the application form:
    • Signed HSG Conflict of Interest Statement
    • Current CV (signed within the last year)
    • Letter(s) of Support (LOS), on institutional letterhead from an HSG Investigator or Mentor
    • Current Medical Professional License
    • Human Subject Protection Program certificate(s) – PHRP Online Training or Collaborative Institutional Training Initiative (CITI) Program or the equivalent
    • Good Clinical Practice (GCP) training certificate- NDAT CTN Training, CITI Program or the equivalent
    • Evidence of UHDRS® training and have completed a minimum of 25 UHDRS® assessments. (the UHDRS® certification is provided and hosted by Enroll-HD. The link for the motor rater training is: https://hdtraining.enroll-hd.org/)
    • (Optional) required if seeking to perform other assessments: Evidence of certification in additional assessments (UHDRS® certifications, CAB, Q Motor, PBA)
    • (Optional) Recent professional head-shot photo

Individuals interested in becoming credentialed as HSG Investigators who are not physicians, must comply with or provide the following in addition to documents in above:

  • There must be a designated backup physician at the site. All backup physicians must agree in writing to serve in the capacity and attest to the following:
    • Acceptance of responsibility for direct medical supervision
    • Must be personally in current good standing with any federally regulated requirements for the conduct of clinical trials.
    • Must submit his/her credentials to the HSG Credentials Committee for review if not currently an HSG credentialed Investigator.
    • Willingness to abide by the HSG confidentiality and conflict of interest policies for the study under which he/she serves.
  • The required letter must be signed and on their institution’s letterhead, addressed to the HSG Committee Chair and submitted with the application materials. This can be included in the LOS, if being written by the same individual.

  • Provide General Information, Site Information, Experience and Training on the application form.
  • Proof of professional license and/or affiliation with institution is in good standing.
  • A potential coordinator should have experience as a coordinator in a clinical trial.
    • If the Credentials Committee finds that the applicant does not have sufficient clinical trial or HD clinical care experience, additional training suggestions will be recommended to the applicant.
  • Must have completed the CME4HD lecture series training (in-person or online)

  • Center involved in the care of individuals with HD
  • Experienced site staff, including HSG credentialed investigators and coordinators

Credentialing Transfer –

Investigators and Coordinators wishing to transfer previously approved credentials to a new site:

  • The relocation should be in good standing and a new Letter of Support should be submitted.
  • Provide General Information, Site Information, Experience and Training on the Transfer application form
  • The following materials must be uploaded with the application form:
    • Credentials Transfer Request Letter (template provided)
    • Current CV (signed within last year)
    • Current Medical Professional License, if applicable
    • Letter of Support on institutional letterhead from an HSG Investigator or Mentor
  • If transferring to a new HSG Site, site will need to submit a new HSG Site application.

Credentialed Member Update – Apply Now!

  • Credentialed HSG Investigators are required to complete an HSG Site Information Update form every three years.
  • Members will be contacted by email requesting the completion of the form.
  • Provide updated contact information and site information on the Site Update form.
